Revolutionising Lung Cancer Diagnosis: The Economic and Clinical Impact of ctDNA Testing in the NHS

Introduction

In 2020, 37,211 people were diagnosed with lung cancer in England. 68% of this population is at an advanced stage and has limited life expectancy. People with advanced lung cancer have complex care needs and often experience high levels of GP appointments, hospital admissions and extended lengths of stay while awaiting diagnosis and treatment.

Timely genomic testing of the tumour can help identify individualised treatments, which can potentially markedly improve the quality and length of life. Delays in identifying specific gene mutations can result in missed opportunities for patients to receive targeted and more effective treatments, ultimately leading to worse outcomes and higher costs for health systems.

Liquid biopsy, a cutting-edge diagnostic method validated through numerous clinical trials, involves testing blood samples for biomarkers like circulating tumour DNA (ctDNA), among others, to detect cancer-related genetic mutations. This less invasive approach offers several benefits, particularly in vulnerable patients with advanced non-small cell lung cancer.

The NHS in England is working towards being a global leader in adopting liquid biopsy testing into a national health service. Recognising the importance of economic assessment and evaluation of the costs and benefits of broader ctDNA testing, Edge Health was commissioned by NHS England to undertake this work to support an ongoing national pilot involving non-small cell lung cancer testing.

Using health economics to understand benefits and costs

Our initial findings in the early phase of the health economics analysis of ctDNA testing combined academic methods with commercial insight and experienced understanding of how the NHS operates to assess the economic implications. This involved collaboration with clinical experts and synthesis of information from various other sources. As a new technology, our analysis considered various clinical scenarios and sensitivities for critical assumptions.

“Implementing ctDNA testing into the routine diagnostic work up of patients with lung cancer is a huge step forward to improving equity of access to state of the art genomic testing for our patients. This will allow patients to receive the best treatment possible for their condition. The input from Edge Health has been invaluable in mapping out a complex pathway, identifying options for ctDNA implementation and their associated cost benefits”.

Outputs from the initial analysis were extrapolated more generally with national data, which helped identify the potential future costs and benefits.

In the context of stage III and IV lung cancer, from early analysis, the application of ctDNA was found to deliver significant benefits relative to its costs. This finding was primarily driven by ctDNA testing enabling earlier blood testing and potentially avoiding tumour genomic testing, which supported patients to access targeted treatments earlier and more consistently – lowering broader system costs. In the next phase of work, pilot data will be analysed to validate these preliminary findings to support the commissioning of the ctDNA test on the genomic national test directory.

Moreover, ctDNA testing is expected to improve equity in genomic testing access substantially, expanding coverage over a broader spectrum of gene mutations and ensuring the inclusion of patients for whom adequate tissue biopsies might not be viable.

Ultimately, incorporating the latest genomics advances into routine healthcare will help deliver the UK government’s vision in “Genome UK: the future of healthcare”.

Demonstrating the value of elective orthopaedic hubs at the NOA awards

With over 750,000[i] people currently waiting for elective orthopaedic operations in England, there has never been a greater need for highly productive units working in partnership. Edge Health was therefore honoured to sponsor and present the Partnerships and Integration Initiative award at the recent National Orthopaedic Alliance Awards event.

The winner of the award, South West Ambulatory Orthopaedic Centre (SWAOC) is the elective orthopaedic hub set up at the renovated Nightingale hospital in Exeter to deliver high-volume and low-complexity (HVLC) activity.

The SWAOC hub was formed from the national initiative to develop 91 hubs across the country and help drive down the backlog in elective orthopaedics. SWAOC won the award for its exceptional results, which included delivering over half (56%) of their activity as same day procedures – a rare, but critical achievement for managing limited inpatient bed capacity.

This outcome was driven by a clinical redesign of the facilities to best align with the hub’s goals and a multidisciplinary team that reviewed international best practice. To do this, the SWAOC team visited exemplar units, engaged with regional clinicians, and developed standardisation pathways of care to deliver better outcomes. As a result, fewer beds are taken up by patients recovering from surgery, reducing the burden on NHS resources more generally.

The elective hub and HVLC programme

Since the start of the COVID-19 pandemic, there has been increasing focus on the elective care backlog that has continued to grow. A solution that emerged from GIRFT in 2021 was the prospect of establishing elective activity hubs, like SWAOC, to rapidly deliver HVLC activity.

Work undertaken by Edge Health to support this programme looked at the opportunity for these hubs, and the capacity required to meet the potential demand. As a result, the policy received over £1.5 billion of funding for the next 3 years and in the last 9 months, over 90 hubs have been set up and are starting to deliver results.

Some of the hubs, like SWAOC, have demonstrated rapid results and now provide a template for others to learn from and follow – both nationally, in the NHS, but also internationally.

What can other orthopaedic departments learn from SWAOC

It is clear that system working, collaboration, and the development of new pathways have been core to the success of SWAOC.

This and SWAOC’s ability to deliver over half of its activity as “same day”, and with only 1% staying longer than 24 hours, has enabled the unit to achieve patient flow. This patient flow has enabled the unit to deliver highly productive results with minimal cost and, arguably more importantly, less demand on people.

Edge has already worked with GIRFT to create a dashboard that enables Trusts to track their HVLC activity. As a result, the 63 orthopaedic hubs already set up in England can identify key areas of improvement in real time that will bring their surgical outcomes in line with the high-performing SWOAC hub.

Study: Impact of travel restrictions on omicron in the UK

Oxera and Edge Health have been commissioned by Manchester Airports Group (‘MAG’) to consider the impact of travel restrictions on the spread of variants in the UK, and to compare this to the impact of domestic restrictions.

The Omicron variant is now highly prevalent in the UK and the government is considering what, if any, further measures should be taken to mitigate the variant’s impact. Our analysis indicates that when a variant is already highly prevalent in the domestic environment, travel restrictions are likely to have a very limited impact on the growth and the peak of cases and hospitalisations. This must be considered alongside analysis undertaken by Oxera in December 2021, which indicated that Omicron travel restrictions could lead to an additional £3.1bn loss in GVA and £1.5bn loss in tax revenue over the next year.

This note sets out the high-level findings from our analysis. The key conclusions are as follows.

Although it is now known that Omicron was circulating internationally throughout November, testing and sequencing of tests for air passengers arriving in the UK over the course of November did not identify Omicron as a Variant of Concern (VOC) in the UK before it was reported by South Africa towards the end of November.

  • Additional testing requirements—i.e. pre-departure tests and Day 2 PCR tests—were put in place by the UK government quickly in late November/early December after Omicron was identified as a VOC. However, these restrictions were ineffective at preventing the spread of Omicron in the UK.
  • Even if these, or other, travel testing requirements had been in place from the beginning of November, they would not have had a significant impact on the spread of Omicron in the UK. Compared to a Day 2 antigen test, a Day 2 PCR and pre-departure testing regime would have delayed the peak of cases by just five days and reduced the peak by only 3%. If no travel restrictions had been in place at all in November/December, cases would have peaked seven days earlier and the peak would have been 8% higher than without any travel testing.
  • Now that Omicron is highly prevalent in the UK, if all travel testing requirements are removed in January, there would be no impact on Omicron case numbers or hospitalisations in the UK, and there would be much lower costs imposed on the UK economy than if the current restrictions are kept in place.

George Batchelor, Edge Health Director:

“New variants of COVID have been progressively more transmissible. Omicron has demonstrated this very clearly by becoming the dominant variant of COVID in the UK between November and December last year, despite relatively quickly imposed travel restrictions. This dominance is due to the highly infectious nature of Omicron, which means community transmission can be seeded from a small number of imported cases – especially true when community restrictions are minimal like now.”

“Our new analysis shows that given the high prevalence of Omicron, continued travel restrictions will not significantly affect the trajectory of Omicron in the UK. This finding is consistent with the Government’s policy of removing countries from the red list on December 15th.”

“Based on the experience from Omicron, our analysis also shows that future travel restrictions for new variants are unlikely to play a significant role in changing the trajectory of infections. This lack of impact is due to the delay between a variant being identified and then being declared a concern.”

The variant of concern

The sun is shining as the UK comes out of its winter hibernation lockdown after a traumatic winter. Yet, despite a hugely successful vaccination campaign and a massive increase in testing, many people remain deeply concerned. Most of this concern surrounds variants to the Wild Type – the strain of the COVID-19 virus with no significant mutations.

There is good reason to be concerned about variants (“Variants of Concern”). The second wave of COVID-19 in the UK has been dominated by the B117 variant, which appears to have originated in Kent. Unlike the Wild Type, B117 is both more transmissible and causes more severe illness. In numbers:

  • without social distancing, each person infected with B117 is likely to infect 3-4 other people compared to 2-3 others for the Wild Type
  • for every 100 +80 years olds infected with B117, 82 will become critically ill, and 16 are likely to die compared to 50 and 10 for the Wild Type

More cases and more critically ill people means more pressure on the NHS, which has triggered the need for both of the major national lockdowns.

The positive news is that the first generation of COVID-19 vaccines is effective at reducing both:

  • people getting infected, which means they won’t get sick or pass on the virus to others; and
  • people becoming critically ill or dying if they do get infected

These effects are good since they mean the risk of a third wave of COVID-19 is reduced, and the impact on the NHS is also reduced. Provided the UK continues its heroic vaccine rollout, and there is some social distancing over the next few months, we should see B117 pushed into submission – just don’t plan on going to any big events.

Given that the B117 variant of COVID-19 is widespread in the UK and under some control, it is not a concern. So how much should we be concerned about other “variants”?

Since the pandemic started, there have been over 137 million reported cases of COVID-19 (certainly a vast underestimate of total actual cases). From this massive number of infections, there have been around 4,000 variants identified. Of these 4,000, only four are currently recognised as Variants of Concern in the UK (four more variants are “under investigation”). Two of these Variants of Concern are homegrown – B117 and B117 with the E484K mutation.*

The two other Variants of Concern are B1351 (South Africa) and P1 (Brazil). Much less evidence is available on these variants – this uncertainty in and of itself creates concern. But at the moment, neither of these variants appears more transmissible nor more deadly than B117. Current evidence also suggests that vaccinations effectively reduce the number of people who get critically ill or die after becoming infected with either of these variants. This impact is tentatively good news – see chart below, which is based on our modelling of the vaccine rollout and current evidence of the different variants.

Based on this evidence, some will conclude the pandemic is over. But for B1351, there is some early evidence to suggest that it can evade vaccinations and still infect people. This ability is terrible since it means the variant could spread through a vaccinated population as social distancing measures are relaxed. Even though fewer people would get critically sick for the same number of infections, there could be a large number of infections (remember that exponential growth quickly makes small numbers very large). The chart below shows this graphically for infectiousness (R0) and the impact of vaccinations – only the B1351 variant sits in the amber zone, meaning it may still spread after the vaccination programme.

The ability to evade vaccines is likely the main reason for surge testing in Lambeth and Wandsworth following an outbreak of at least 44 cases of B1351 at a care home.

The next few weeks will be critical for understanding B1351 in greater depth, specifically on how much it evades the current generation of vaccinations. It will also be essential in seeing how it progresses in the battle for COVID-19 dominance, which as the chart below shows, is a race currently being won by B117 (Kent).

Without going into the genealogy (not my area) too much, there are a limited number of mutations that can benefit the SARS-CoV-2 virus. Consequently, as I understand there is likely to be a high degree of convergent evolution – this is why we have only variants coalescing around the same mutations.
