Case-mix adjustment of NHS patient outcomes

Recently Edge health was retained by a large NHS programme in the UK to support the development of an approach to risk adjusting surgical outcome data. The client in question currently reports surgical outcome data back to practicing surgeons (confidentially) to aid them with their revalidation as well as quality improvement.

Why was risk adjustment of surgical outcome data important for our client?

Consider the following scenario, comparing two consultants. Consultant A has a very low mortality rate (0.2% of patients on which this consultant operated on sadly died), while Consultant B has a higher mortality rate (3.2% of patients undergoing the same procedure with this consultant sadly died). These “raw” rates do not consider the patient characteristics, or underlying comorbidities (case-mix) of the patients treated by each consultant.

For example, Consultant A may have seen very young, healthy patients, while Consultant B has seen older patients with more comorbidities such as heart conditions or diabetes. Despite undertaking the same surgical procedure in theory it would be unfair to say Consultant B actually performed worse.

A proper risk adjustment approach therefore has to take into account the underlying patient characteristics to adjust important outcome measures. Here, in our example, given the underlying patient characteristics, Consultant B actually performed better than Consultant A (after risk adjustment).

The resulting model

We then developed and validated a risk adjustment model that used regression based methods to predict expected outcomes based on the observed data and used this to adjust the outcomes at surgeon level. We also implemented machine learning methods to determine the performance of our model. This was especially important in this instance, as the smaller consultant level samples make it harder to evaluate performance. A key risk for us was that at individual surgeon level, risk adjustments can quickly become unstable or unreliable due to the low number of observations. It was important to present only statistically stable, reliable, risk-adjusted rates. We thus had to determine a dynamic lower bound on the number of observations required per consultant for risk adjustment, balancing consultant coverage and statistical validity.

For this project, we presented the risk adjusted rates in funnel plots with control limits (labelled alert and alarm for research purposes only) (Figure 2 below). The data is now used with selected clinicians on a confidential and voluntary basis with the goal to undertake detailed evaluation and testing and with clinician support ultimately make it available to all consultants in the country.

Context 

Since Spring 2020, international air travel has been severely impacted by COVID-19 and associated restrictions. Restrictions that were introduced around the globe often involved multiple tests and quarantine requirements. Restriction regimes led to a substantial decline in travel often keeping families separated at difficult times. For those that did travel, the cost of travel testing has been substantial, often complex, and occasionally leading to significant disruption. 

A challenge for the aviation sector and governments across the globe has been to understand the rapidly evolving evidence base, and to inform life changing policy decisions.  

Our support

Since 2020, a collaboration between Edge Health’s health and data science team with Oxera’s aviation experts have provided independent support to the aviation sector to help develop a robust understanding of the evidence on international travel restrictions. In early 2022, our work on the limited value of restrictions and testing led to the removal of all restrictions in UK and is now directly influencing the removal of restrictions across the globe.  

History of our support

In late 2020, landmark work from Edge Health and Oxera led to the removal of the UK’s blanket 14 day quarantine restrictions for international travelers.
This quarantine restriction was replaced by a greater reliance on COVID testing – a trend seen across the globe in 2021. The policy towards testing evolved over 2021 with support from Edge Health and Oxera, which in general sought to clarify the diminishing value of multiple tests. 

It was not until early 2022 that the true extent of the limited value of international travel restrictions became clear in the aftermath of the Omicron wave of infections – a wave of infections totally unaffected by the extensive international travel restrictions in place at the point in time.
Since early 2022, Edge Health and Oxera have provided similar insight and support across Europe, and most recently for Japan in Asia.

Omicron and the NHS

Despite relatively rapid travel restrictions, Omicron will soon become the dominant variant of COVID in the UK. It has been able to achieve this by reinfecting people that have previously had COVID or vaccinations – hence it is best described as a “sneaky” variant.

There is positive anecdotal evidence that Omicron is “far milder“. Still, MUCH higher case numbers might translate into unacceptably high hospital admissions.

The critical question is: can the NHS cope with this wave of COVID without a national lockdown?

Recent modelling from LSHTM suggests that hospitalisations could peak in late January 2022 at levels between half and double the January 2021 peak (see chart below – dotted lines show January 2021 peak).

This modelling leaves a lot of room for uncertainty. Still, as some excellent analysis from James shows (here), it currently looks most likely that the Omicron wave would result in a similar level of hospitalisations as the Alpha wave last January – albeit at a potentially quicker rate.* This better case scenario is due to vaccinations and previous infections protecting against severe disease and hospitalisation. Boosters will also have a significant effect.

No analysis has yet to consider the additional impact of molnupiravir; the antiviral approved for use in the UK in November. Early evidence for molnupiravir suggests that it reduces hospitalisations between 30% – 50%. Combined with the above analysis on possible hospitalisations, the Omricon wave could be significantly lower than the Alpha wave of January 2021 – possibly half as many admissions with a shorter length of stay, so even fewer beds (my semi-scientific guesstimate).

Unfortunately, the NHS was stretched pre-COVID and has been pushed to its extremes during COVID and the recovery. Staff are exhausted and some traumatised. As my GP friend put it: “honestly, everyone is shattered…it’s unsafe in lots of places”. This triangulates with anecdotal evidence that sickness rates are twice as high compared to this time last year – the chart below shows NHS sickness data over the previous three years up to August 2021.

Add to all of this:

  • a likely spike in NHS staff sickness due to (re)infection with COVID
  • a general increase in NHS pressure (which would have been rising even without the pandemic)
  • a growing backlog of ‘non urgent’ activity that is rapidly becoming ‘urgent’; and
  • a (bizarrely timed) reorganisation

and the NHS may well face an even more challenging test in January 2022 than 2021!



Outpatient video consultations in secondary care

Outpatient video consultations can deliver a range of benefits to patients and the NHS, according to a research report by Edge Health published today 14 September 2021.

The report reviews 3 million video consultations over a year in one of the fastest roll-outs of a single technology in the NHS’s history. This took place in a matter of weeks across 171 NHS hospital trusts in England, involving more than 69,000 consultants and clinicians using a single NHS supported video platform.
 
Video consultations with patients increased from around 5,000 a month in March 2020 to 340,000 a month a year later in a wide range of areas, including mental health, orthopaedics, paediatrics, physiotherapy, neurology, maternity and reproductive medicine, respiratory, diabetes, ophthalmology and oncology.

The report highlights the benefits of video consultations to patients – saving over the year a total of 4.64 million hours (530 years) inpatient travel and in-hospital waiting times and £40m in travel costs and parking charges. As a result, 3 million lost work hours were avoided saving the overall economy £108m in lost productivity. The report estimates fewer journeys to the hospital resulted in 14,200 tonnes of avoided greenhouse gas emissions, and 11 million fewer “single-use” PPE items, such as face masks, were consumed, saving the NHS over £1.1 million. The analysis also found that 1,730 hospital-acquired infections were avoided (not including COVID-19 infections) due to fewer visits to hospital sites. 
 
Over 76% of clinicians were “positive” or “very positive” about video consultations with patients, 77% finding the national video platform “easy to use”. Research conducted earlier this year by Oxford University for NHS Scotland has also shown very high patient satisfaction rates with NHS video consultations.
 
Edge Health’s research also found in its regional analysis high levels of usage across a range of geographies suggesting broad patient appeal in both rural and urban areas.

Only by showing patients the benefits of video consultations and backing this up with the necessary support to access care in this way will we be able to safeguard the progress made and realise further opportunities in how video technology can improve patient care

Study: Safe list could be more than doubled with no increase in risk, due to UK vaccinations

  • Findings come as the UK government considers the reopening of international travel from May 17 with a suggested tiered approach to managing risk

The success of the UK’s vaccination programme means the number of countries on the travel ‘green list’, could be more than doubled without putting public health at risk, research ahead of the Government’s Global Travel Taskforce report shows.

A study carried out by Edge Health and Oxera for Manchester Airports Group shows that if case rates this summer are similar to last year, passengers could start travelling to more than 130 countries such as Spain, the United States, and Dubai.

The study looks at the risk of new infections from July and August 2020, when the travel corridor scheme allowed people to visit countries in which rates were below a certain threshold.

The Edge Health and Oxera modelling shows that because of the extent of the UK’s vaccination roll-out, travellers from the UK could now visit overseas destinations with virus prevalence rates three times higher than they were in 2020, without increasing the risk or putting pressure on the NHS.

This is because vaccinations increase immunity in the population, drastically reducing severe illness and also leading to a lower rate of transmission (R number).

If the vaccines had been available in 2020 this would have safely increased the number of countries on the list from 62 to 102.

If pre-departure high-performing antigen tests are administered, prevalence could increase by a factor of six to eight with no further impact to risk.

That would mean almost 30 additional countries could be added to the list, letting passengers travel safely to up to 91% of all countries.

When vaccination programmes in other countries outside of the UK and the vaccination of those aged 17 and below is taken into consideration, the number of possible destinations could rise even further.

The study has been combined with data that shows the success of the UK’s vaccination programme has led to a significant reduction in hospitalisations and deaths from COVID-19. This demonstrates that an increase in the number of ‘green list’ countries is possible whilst maintaining the low levels of new infections observed in Summer 2020.

Edge Health and Oxera’s and study is based on a list of the 143 countries where estimates of COVID-19 prevalence are available and includes the top ten destinations for passengers from the UK.

‘Although the risk of variants remain a concern none have yet become dominant over the wild type of SARS-CoV-2 globally. In the UK, where the dominant variant is now B117 (Kent), vaccines have been shown to be effective which means we can be cautiously optimistic. This means international travel can be safely restarted if travel policies, such as testing, are targeted at countries with greater levels of risk. To the extent that other variants need to be treated carefully in coming months as we learn more about their characteristics, it is important to remember that many of them are already present in the community.

‘The successful rollout of the UK’s vaccination programme, with 32m people having received a first dose, is crucial to the safe re-opening of air travel. The vaccines’ role in significantly reducing not only the number of hospitalisations and COVID-19 deaths but transmission rates of the virus should give assurance that safe travel to international destinations is within reach.’

Study: Single COVID-19 test on arrival as effective as ten-day quarantine

A single on-arrival antigen test is as effective as a ten-day self-isolation period in reducing imported cases of COVID-19, new modelling has revealed. Based on prevalence rates at home and abroad and accounting for quarantine compliance, Edge Health and Oxera find that a single rapid antigen test would be appropriate for arrivals from the majority of countries categorised as medium risk, where there are medium caseloads or lower vaccination rates. Such countries could be designated as ‘amber’ if the UK Government applies a traffic light system for foreign travel to categorise risk.

In evidence submitted to the UK government’s Global Travel Taskforce this week, many of the quick, inexpensive and effective antigen tests are shown to reduce the number of infectious days of an international arrival by 63%. This means that the risk of community transmission from a single test is comparable, if not more effective, to a ten-day quarantine period with no testing, which screens 62% of infectious days and was the UK border policy for countries without travel corridors until 18 January 2021.

The new findings demonstrate how the government’s evidence base has underestimated the effectiveness of a single antigen test, which SAGE previously reported would screen only 11% of infectious travellers.

For higher-risk countries, a two-test strategy could be a viable option as part of a risk-based framework to safely reopen international travel this summer. Using a 72-hour pre-departure antigen test and three-day quarantine with PCR test, 87% of potentially infectious days are screened—just one percentage point lower than a dual-PCR test. Crucially, the new evidence demonstrates that the two-test approach delivers effective levels of screening, while ensuring that the UK can easily track and genome sequence the tests of passengers, mitigating the risk of importing variants of concern.

The government currently mandates a pre-departure test within 72 hours of travel to England and managed quarantine in a hotel for ten days with two PCR tests for anyone arriving from a red list country, at a cost of £1,750. Arrivals from all other countries must quarantine at home for ten days and purchase a £210 travel test package with two PCR tests, for administration on days two and eight of self-isolation.

‘Given the success of the UK vaccination programme, and in line with the UK government’s risk-based approach to the wider economy, any restrictions should be targeted at reducing the potential import of variants of concern. Our modelling demonstrates that a single antigen test for medium-risk countries and dual-test approach for higher risk countries, combined with three-day quarantine, could be an effective strategy to protect public health, while removing the burden of a ten-day quarantine.’

‘The ultimate goal for governments must be to return to frictionless travel between countries that are considered lower-risk. The vaccine roll-out, combined with internationally agreed standards on digital solutions to evidence COVID-status certification, will be intrinsic to that, but people should be able to travel safely, with or without a vaccine.

‘Our analysis of the effectiveness of different quarantine and testing strategies shows that the UK government can safely restart international travel at scale while tailoring test requirements and restrictions on a country-by-country basis.’

Comparison of dual- and single-testing schemes finds only a marginal benefit, with the two-test approach screening just 5–9% of additional infectious days compared to one test. For example, a single antigen test administered on departure screens 62% of infectious days, while two antigen tests, one administered 72 hours before departure and another on departure, screen 67% of infectious days.

Using 2020–21 data from the USA and the EU, the report also finds that even when COVID-19 prevalence has been higher than the UK, arriving air passengers contribute fewer potentially infectious days into the community per capita compared to domestic infections.

Based on real-world data from 14 December 2020, when prevalence in the USA was the highest that it has been relative to the UK, 10,000 incoming air passengers would have had 79% fewer infectious days than 10,000 individuals in the UK community if a single on-departure antigen test had been administered. The relative risk of importing infections is also expected to decrease further as vaccinations are rolled out domestically and internationally. Around 31 million total Covid-19 vaccine doses have now been administered in the UK, with more than half of adults receiving a first dose.

The report has been submitted to the UK government’s Global Travel Taskforce to inform its new risk-based framework to facilitate international travel.
